Yale New haven Health (YNHH) is seeking a compassionate, board-certified (or eligible) Hospice & Palliative Care Physician to join our highly respected hospice and palliative care program. You will work with an interdisciplinary team to deliver patient-and-family-centered care for individuals with serious life-limiting illness, both in the home and inpatient settings. Requirements

  • MD or DO from an accredited medical school
  • Board certified or board eligible in Internal Medicine or Family Medicine
  • Active or eligible for Connecticut medical license

Nice To Haves

  • Fellowship trained and/or Board Certification in Hospice & Palliative Medicine preferred

Responsibilities

  • Provide consultative and direct patient care in palliative and hospice settings (home, skilled nursing facility, inpatient)
  • Lead goals-of-care discussions with patients, families and the care team
  • Manage complex symptom burden (e.g. pain, dyspnea, nausea) and coodinate care plans aligned with patient values.
  • Collaborate with physicians, advanced practice providers (APPs), nursing, social work, spiritual care and case managers.
  • Participate in quality improvement initiatives, program development, and education of medical staff.
  • Support transitions from curative to hospice-focused care when appropriate
