Join us in our efforts to better our community! As the Hospice Patient Care Manager, you will supports the Administrator in planning, coordinating, and overseeing the daily clinical operations of the Blue Skies of Texas Home Health Agency and Hospice program. This role is responsible for coordinating and supervising an interdisciplinary care team to ensure the delivery of consistent, high-quality hospice services within the assigned service area. Care is provided in accordance with physician orders, established plans of care, and all applicable federal, state, and regulatory requirements. The Hospice Patient Care Manager plays an active leadership role in quality assurance and performance improvement initiatives to promote excellence in patient care and program outcomes. What you will do in this role:

Requirements

  • This role requires strong attention to detail and a commitment to delivering exceptional customer service to residents, employees, families, outside agencies, and all individuals encountered in the course of daily responsibilities.
  • The position requires the ability to identify required policies and procedures, develop and implement them effectively, educate clinical staff, and ensure ongoing compliance with all applicable standards and regulations.
  • The candidate must be able to speak, read, write, and comprehend instructions in English.
  • Current Registered Nurse license in good standing with the State of Texas or a compact state permitting practice in Texas.
  • A minimum of one (1) year of management or supervisory experience in a healthcare-related setting.
  • Demonstrated experience with budgeting or financial oversight responsibilities.
  • Current certifications in Cardiopulmonary Resuscitation (CPR), First Aid, and Automated External Defibrillator (AED).
  • Valid Texas driver’s license with an acceptable driving record and remain insurable by Blue Skies of Texas insurance carriers.
  • Participation in continuing education activities as required by applicable regulations and the Texas Board of Nursing.

Nice To Haves

  • National certification in Hospice or Home Health nursing.
  • Registered Nurse, BSN, with three (3) to five (5) years of leadership experience in Hospice and Home Health programs.
  • Experience in staff development, mentoring, program development, and service expansion is strongly preferred.
  • Knowledge of Medicare regulations and reimbursement is highly desirable.

Responsibilities

  • Assists with the referral and admission process of intake, assignment, collecting insurance information in an effective and efficient manner; fill in for Admissions as needed
  • Manage all direct patient care by assigning staff to patients, scheduling staff for maximum budgetary optimization including PTO and on call, resolve patient care issues, visit patients as needed, coordinate interdisciplinary group.
  • Audit patient records and responds appropriately to compliance issues and write compliance or QA reports.
  • Maintain compliance with all regulatory and accrediting agencies.
  • Manage clinical staff through effective hiring, performance evaluation, coaching, staffing, scheduling and delegation.
  • Educate, counsel and orient staff to Agency policies and procedures.
  • Ensures standards of ethical business and clinical practice are maintained.
  • Ensures the structure and systems promote interdisciplinary care.
  • Assurance of adequate and appropriate staffing.
  • Other duties as assigned by the Chief Health Services Officer.
