Empyrean Hospice hires On Call RNs (Registered Nurses) to provide on-call coverage as scheduled and assist RN Case Managers by conducting scheduled after-hours visits as necessary to meet patient care needs. Represent Empyrean Hospice with the utmost professionalism and compassion. Complete an initial, comprehensive, and ongoing comprehensive assessment of patient and family to determine hospice needs. Provide a complete physical assessment and history of current and previous illness(es). Initiate and develop an individual care plan that establishes goals that includes the patient and the family in the planning process. Assume responsibility to coordinate patient care for assigned caseload. Counsel the patient and family in meeting nursing and related needs and provide health care instructions to the patient as appropriate. Assist the patient with the activities of daily living and facilitate the patient’s efforts toward self-sufficiency and optional comfort care. Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given. Communicate with the physician and hospice care team regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physician orders as required; and work to provide continuity of care. Provide and maintain a safe environment for the patient and arrange equipment and other necessary items and services as needed. Supervise ancillary personnel and delegates responsibilities when required. Ensure standards of ethical business and clinical practice are prioritized.

  • Understanding of hospice care philosophy
  • Current licensure in State and CPR certification
  • Graduate of an accredited school of nursing with 1-2 years of recent acute care experience
  • 1-2 years of recent experience in home care
  • Bachelor’s degree preferred with one year of hospice care experience
  • Excellent observation, verbal and written communication skills
  • Basic math skills and nursing skills per competency checklist
  • Ability to lift, position, and/or transfer patients
  • Licensed driver with an insured automobile in good working order
  • Willingness to maintain knowledge of information systems and software
  • Management experience preferred but not required
