About The Position
We are seeking a compassionate and experienced Advanced Practice Provider (APP) to join our dedicated Hospice Care team. In this role, the APP will collaborate closely with the interdisciplinary hospice team to deliver high-quality, patient-centered end-of-life care. Responsibilities include providing direct patient care through comprehensive assessments, expert symptom management, and individualized treatment planning, while also serving as a clinical resource for hospice staff and external care teams to support continuity and excellence in care.
Requirements
- Must be BE/BC and able to obtain a MN License.
- Current licensure as a Nurse Practitioner (NP) in the state of Minnesota
- Valid driver’s license and reliable transportation
Nice To Haves
- Experience in Hospice or end of life care preferred
