About The Position

The Nurse Practitioner will play a critical role in delivering comprehensive healthcare services to diverse patient populations in our hospice care. This position focuses on providing expert clinical assessments, managing pain effectively, and ensuring patient safety while upholding the highest ethical standards, particularly in end-of-life care. The Nurse Practitioner will collaborate closely with interdisciplinary teams to develop and implement individualized care plans that address both acute and chronic health conditions. Through patient education and advocacy, the role aims to improve health outcomes and enhance quality of life for patients and their families. Ultimately, the Nurse Practitioner will serve as a trusted healthcare provider who combines clinical expertise with compassionate care to meet the complex needs of patients in the United States healthcare system.

Requirements

• Current STATE RN and Advanced Practice Nurse License
• Valid Driver’s License
• Valid Auto Insurance
• AHA CPR Certification

Responsibilities

• Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
• Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
• Perform Face-to-Face visits at the patient's place of residence.
• Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physicians.
• Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
• Keeps up with and maintains education/knowledge in pharmacology and palliative medicine.
• Knowledgeable of hospice criteria.
