About The Position

POSITION SUMMARY: The Advanced Practice Provider - Hospice provides consultation in palliative care, symptom management and supportive care to meet the needs of the Hospice patient as requested by the referring provider or Hospice Medical Director. The position is an integral component in facilitating clarification of patient and family goals of care. DUTIES & RESPONSIBILITIES: Makes face-to-face encounter visits within prescribed time limits to Hospice patients and reports clinical findings to the Hospice Medical Director, primary provider, and Hospice team. Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities. Consults with Hospice Medical Director as needed, informs primary provider of any recommended services, and collaborates with other providers as needed. Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to Hospice care. Collaborates with the patient/family, attending provider and other members of the IDG in providing patient and family care. Instructs and supervises the patient/family in self-care techniques when appropriate. Maintains accurate and relevant clinical notes regarding the patient’s condition. Reports patient condition and any identified needs/issues to Hospice Director and Hospice Medical Director. Can supervise and evaluate the home health aides ability to perform assigned duties. Order diagnostics / treatments as needed after collaboration with Hospice team and referring providers. Participates in Interdisciplinary Group Meeting (IDG) regarding patient care. Participates in the QAPI plan and process. Assists in development of clinical practice guidelines/standards in support of quality palliative and hospice care as requested by supervisor. Prescribes medications including controlled substances to the extent delegated and licensed after collaboration with Hospice team and provider. Dress in a professional manner. Display good interpersonal skills with the patients and staff. Maintains patient confidentiality at all times. Assure proper follow-up on all abnormal test and x-ray results. Maintain the necessary accreditation on required certifications. Demonstrate appropriateness in meeting objectives in age-specifics. Perform other duties as assigned.

Requirements

  • Graduate of an accredited advanced practice practitioner program.
  • Current certification as a Clinical Nurse Specialist or Nurse Practitioner in the state of Michigan
  • Current certification by a national body such as ANCC, NBCHPN or AANP.
  • Current DEA license preferred.
  • Current CPR certification
  • Minimum 5 years Hospice experience preferred.
  • Palliative Care ARNP preferred.
  • Excellent clinical skills.
