Job Summary and Responsibilities As a Nurse Practitioner with CommonSpirit Health at Home, you’ll provide face-to-face assessment services to patients to assist the Medical Director and team to determine each patient’s continuing eligibility for home care or hospice care. This role may involve providing services in various settings, including the patient’s home, a physician’s office, or an inpatient hospice facility. You will support our mission to exceed expectations in healthcare delivery, guided by our values and commitment to customer service and our People First Behaviors.

Requirements

  • Graduate of an accredited school of nursing.
  • Currently licensed to practice in the state serviced, with a Master's in Nursing.
  • At least 1 year of nursing experience.
  • Must obtain and maintain current CPR Certification.
  • Valid driver’s license and auto insurance coverage according to the CommonSpirit Health at Home policy.
  • Compliant with appropriate state certification/registration and CommonSpirit Health at Home requirements as necessary.

Nice To Haves

  • One year of hospital nursing experience preferred.

Responsibilities

  • Supports CommonSpirit Health at Home's mission, values, strategic goals, and high standards of customer service, consistently living People First Behaviors.
  • Adheres to the Corporate Compliance Program, including confidentiality of HIPAA protected health information.
  • Provides care following the plan of treatment and patient care plan to assist in the determination of hospice eligibility.
  • Establishes a care plan for continued needs based on the client assessment.
  • Reports changes in patient condition to appropriate personnel and the attending physician.
  • Provides patient care education, counseling, medication advice, instructions, and follow-up to clients and other caregivers.
  • Participates in coordination of services with other disciplines and community resources as needed.
  • Evaluates and revises care plans/assessment sheets based on changes in patients and/or the environment.
  • When providing private duty or staffing in a facility, follows the sign-in procedure and the policies and procedures for that particular institution.
  • Presents patient assessment information to the supervising physician.
  • Performs appropriate emergency measures as deemed necessary.
  • Assists with the development of educational programs in the hospice program for diverse age groups (i.e., infancy, childhood, adolescence, adult, and geriatric).
  • Provides paraprofessional supervision as required by policy and regulations.
  • Completes and submits all required documentation in a timely manner.
  • Regulates and adjusts medications as indicated.
  • Demonstrates sensitivity to patient and/or family needs, customs or feelings.
  • Participates in case conferences.
  • Recognizes emergencies and has knowledge of emergency procedures.
  • All other duties as assigned.
