About The Position

Good Samaritan Hospice, a mission of Concordia Lutheran Ministries, provides compassionate home care, facility based, and inpatient hospice services to our patients and their families. Our home care coverage area spreads throughout several counties in Western Pennsylvania with inpatient locations in Wexford, Cabot, and Beaver. The Good Samaritan Hospice team works together to provide relief through pain control and symptom management, while offering emotional, social and spiritual support for the patient and family. Hospice Nurse Practitioner Position: Are you a compassionate, skilled clinician who feels called to make a difference in the lives of patients and families facing end-of-life care? Good Samaritan Hospice, an entity of Concordia Lutheran Ministries, is seeking a Hospice Nurse Practitioner to join our dedicated team. As a valued member of our hospice care team, you’ll provide expert clinical support, symptom management, and compassionate guidance to patients and their loved ones. You’ll collaborate with physicians, nurses, social workers, and chaplains to ensure comfort, dignity, and quality of life are always at the heart of what we do.

Requirements

Current Nurse Practitioner license (CRNP) in the state of Pennsylvania.
A compassionate, patient-centered approach to care.
Excellent communication and collaboration skills.

Nice To Haves

Hospice and/or palliative care experience preferred.

Responsibilities

Provide hospice patient assessments, symptom management, and clinical direction in collaboration with the hospice medical director.
Work closely with the interdisciplinary team to create and adjust individualized plans of care.
Offer education and emotional support to patients and their families.
Participate in team meetings and ongoing quality care initiatives.
