About The Position

The Hospice NP provides comprehensive clinical care to patients with advanced or terminal illness, offering routine and emergency assessments, symptom management expertise, and compassionate support to patients and families. They collaborate closely with hospice physicians, nurses, and the interdisciplinary team, conduct face-to-face visits to ensure continued hospice eligibility, and help maintain patient comfort and coping at end of life. The NP also serves as an educational resource for staff, caregivers, and community partners while upholding hospice standards, Medicare Conditions of Participation, and organizational guidelines. This is a temporary role expected to run from May 2026 through October 2026 to cover a parental leave period.

Requirements

  • Nurse Practitioner with a current nurse practitioner license in State of Colorado, in good standing.
  • Previous Hospice Experience
  • 2+ years experience in the care and treatment of patients diagnosed with serious, advanced or terminal illnesses is preferred.
  • Current CPR certification
  • Valid Driver’s License and evidence of insurance coverage.
  • Professional demeanor and appearance.
  • Excellent verbal and written communication skills.
  • Demonstrates organizational and time management skills
  • Demonstrates understanding of business needs and leadership skills.

Nice To Haves

  • 2+ years experience in the care and treatment of patients diagnosed with serious, advanced or terminal illnesses is preferred.

Responsibilities

  • Provides medical expertise on pain, symptom management, medications (including formulary guidance), wound care, and evaluation of terminal prognosis to patient, family members, facility staff, and IDG team members.
  • Orders appropriate therapies, medications, and tests when needed
  • Provides education to Dignity staff and community partners within hospice service area.
  • Interacts with attending physicians, as necessary, regarding the care and treatment of patients.
  • Performs patient visits primarily in private residences, long-term care facilities, assisted living facilities, and other locations.
  • Reviews admissions for hospice eligibility; oversees and directs recert/discharge process; reviews long length of stay patients periodically.
  • Participates and advises in IDT meetings
  • Participates in Provider on call rotation
  • Completes certifying forms for family members as needed such as FMLA documentation, facility moving forms, excuse notes, MOST forms (review & sign) etc.
  • Acts as the attending physician for hospice patients (when chosen by patient).
  • Prescribes medications including controlled substances to the extent delegated and licensed.
  • Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
  • Responsible for establishing and maintaining positive relationships with customers and referral sources within hospice service location; responding to customer and community needs regarding education as it relates to the hospice philosophy of care and delivery of services.
  • Attends required education or provides evidence of 12 hours per year of continued education related to end of life care.
  • Ensures that the care being provided is based on established hospice care guidelines, Medicare Conditions of Participation, and standards (NHPCO).
  • Performs other duties as assigned.
