About The Position

The Hospice Nurse Practitioner role at Phoenix Home Care & Hospice involves providing compassionate care to patients in the Springfield, MO area. Requirements

• Completed an accredited Nurse Practitioner Program
• Missouri or Compact nursing license
• Hospice or Long-Term Nursing experience
• Maintains controlled substances registration with state and federal authorities, if applicable
• Not excluded from the Medicare program
• Valid driver's license and auto liability insurance

Responsibilities

• Complementing attending physician care
• Acting as a medical resource person to the IDG
• Performing Face-to-Face visits as needed
• Evaluating hospice eligibility as assigned in consultation with the hospice interdisciplinary team
• Assuring that the patient receives appropriate measures to control uncomfortable symptoms
