What You’ll Do As a Nurse Practitioner specializing in in-home and hospice care, you’ll have the flexibility to manage your schedule while delivering personalized, dignified care to patients and families during life’s most important moments. Conduct in-home visits, physical assessments, and face-to-face evaluations Develop and manage individualized care plans in collaboration with physicians and care teams Prescribe medications and treatments, ensuring symptom control and comfort Educate and support patients and families through end-of-life care decisions Serve as a trusted resource for hospice eligibility and recertification What We Offer Flexibility: Enjoy autonomy in managing your visits and balancing work-life needs. Requirements

  • Master’s degree and completion of an accredited Nurse Practitioner program
  • Current state licensure and CPR certification
  • At least 1 year of NP experience and 1 year in hospice or palliative care
  • Strong communication, problem-solving, and clinical skills
  • Valid driver’s license, auto insurance, and reliable transportation

Nice To Haves

  • Certification in Hospice and Palliative Nursing (CHPN) or specialty areas such as geriatrics or pediatrics preferred, but not required
