About The Position

Santa Maria Hospice RN Case Manager is a professional nurse with ethical solid and empathetic nursing standards. Our case manager must work with attending physicians and hospice medical directors while serving as an educator and guide to the palliative care staff. The RN Case manager must have extensive experience coordinating, documenting, and participating in palliative care plans. Offers comprehensive nursing evaluation with appropriate documentation Excellent bed-side manners and the ability to perform patient visits as needed Assesses for signs or symptoms of discomfort followed by prompt action Provides an appropriate plan of care and supervises caregivers in managing patient services programs Coordinates all services with patients/loved ones to ensure awareness and quality of care Supervises the implementation or modification of the approved plan of care under physician guidance Supervises home-health aide and LVN/LPN Comfortable working in a flexible scheduling environment to ensure patient readiness Maintain knowledge of state and federal regulations and JCAHO standards (if applicable) Participates in the QAPI program Eliminates or reduces safety hazards throughout patient and working environments Complies with hospice policies, procedures, and regulation laws or standards Legible solid, and accurate documentation skills/standards Attends and participates in staff-meeting and patient care conferences

Requirements

  • Graduate of an accredited nursing school
  • Current RN license within the state of CA
  • Ability to work on-site, Santa Maria, CA
  • Minimum of one-year experience as a professional nurse within the last three years or have a Baccalaureate in Nursing from a program accredited by the National League of Nursing
  • Valid driver's license and auto insurance

Responsibilities

  • Offers comprehensive nursing evaluation with appropriate documentation
