Position Overview As a Hospice Case Manager (RN), you will coordinate and provide high-quality, patient-centered care in patients’ homes or care facilities. You’ll work independently while collaborating closely with an interdisciplinary team to support patients and families physically, emotionally, and spiritually. Requirements
- A graduate of an approved school of nursing (RN/BSN/ARNP) with a current Iowa Nursing License.
- Two years of home health or hospice or work related experience preferred. General medical knowledge related to home health and hospice services with end-of-life issues is desirable.
- Car, driver's license and adequate insurance as outlined in the personnel policies is required.
- Strong clinical knowledge, critical thinking skills, and evidence-based practice expertise.
- Excellent communication, interpersonal, and organizational skills.
- Ability to prioritize tasks, delegate responsibilities, and manage multiple priorities effectively.
- Proficient computer skills, including electronic health records (EHR) and Microsoft Office applications.
- Advanced Certifications in Hospice and Palliative Nurse certification by the 3rd year of employment.

Responsibilities
- Develop, implement, and manage individualized plans of care
- Coordinate medical, emotional, and spiritual support services
- Monitor patient condition and adjust care plans as needed
- Educate patients and families on hospice services and symptom management
- Collaborate with physicians, nurses, social workers, chaplains, and other care team members
- Maintain accurate and timely documentation in the EMR
- Participate in interdisciplinary team meetings and quality improvement initiatives
