We invite you to become part of Connecticut’s most comprehensive healthcare network as a Hospice Navigator/GIP (General Inpatient) Hospice RN. The Hospice Navigator/GIP (General Inpatient) Hospice RN is a hospital-based hospice representative responsible for conducting goals of care conversations, identifying appropriate hospice patients, facilitating timely placement into the correct level of care, and ensuring smooth hospital-to-home transitions whenever possible. The role includes educating patients, families, and providers on hospice philosophy and levels of care, evaluating patient needs, and serving as a subject matter expert on home hospice services and GIP eligibility requirements. When symptom management cannot be handled in the community, the nurse provides ongoing inpatient care and ensures patients meet Medicare hospice guidelines for GIP stays.

Requirements

  • Associate’s Degree in Nursing required.
  • RN with 3-5 years acute care experience required.
  • Licensed Registered Nurse in the State of CT required.
  • Critical thinking and triage skills necessary in a facility environment.
  • Ability to work with a variety of care professionals

Nice To Haves

  • Bachelors of Science in Nursing (BSN) preferred.
  • Experience Home care and Hospice experience with General Inpatient understanding preferred.

Responsibilities

  • Educate hospitalized patients and families about the home hospice benefit and support their decision to enroll, while facilitating referrals and ensuring smooth transitions from the hospital to home hospice care.
  • Assess patients for eligibility and admit them to General Inpatient (GIP) level of care when appropriate and conduct ongoing reassessments for patients receiving GIP services.
  • Collaborates with hospital staff and the hospice interdisciplinary team to develop and adjust care plans, providing symptom management recommendations to ensure high-quality end-of life care.
  • Assess inpatient and emergency department patients for hospice eligibility and evaluate GIP patients for possible transition to home hospice care.
  • Maintain a visible presence in the ER and inpatient units to build relationships, increase hospice awareness, and encourage early referrals for hospice evaluation.
  • Perform other related duties as required.
