About The Position
We are seeking a compassionate Per Diem Hospice Medical Social Worker (MSW) to support hospice patients and families by providing psychosocial assessments, counseling, and resource coordination as part of an interdisciplinary team. Requirements
- MSW from an accredited program
- Current CA LCSW or ASW
- Valid CA driver’s license & reliable transportation
Nice To Haves
- Hospice or healthcare experience preferred
Responsibilities
- Complete psychosocial assessments
- Provide emotional support, counseling, and crisis intervention
- Assist with advance care planning and community resources
- Collaborate with nurses, chaplains, and IDT members
- Document visits accurately and timely per hospice regulations
