About The Position

Hospice of South Texas (HOST), a mission-driven, non-profit hospice organization, is seeking a seasoned physician leader to serve as Hospice Medical Director. This is a high-impact executive clinical leadership role responsible for ensuring the integrity, quality, and compliance of hospice care across all service lines. This position offers the opportunity to shape clinical standards, influence organizational strategy, and lead physician practice within a respected, community-based hospice known for delivering 5-star quality care. The Medical Director works in close partnership with the Chief Executive Officer and executive leadership team, serving as the clinical authority guiding patient care, provider performance, and regulatory excellence.

Requirements

  • Doctor of Medicine or Doctor of Osteopathy with an unrestricted Texas medical license
  • Board certification in a relevant specialty
  • Active DEA registration (Schedule II authority)
  • Active Medicare and Medicaid participation
  • Strong executive presence and decision-making capability
  • Ability to influence physician practice and interdisciplinary teams
  • Deep understanding of hospice regulations, compliance, and audit readiness
  • Excellent communication skills for complex goals-of-care discussions

Nice To Haves

  • Hospice and/or palliative care experience
  • Hospice Medical Director Certification or board certification in Hospice & Palliative Medicine
  • Prior experience in physician leadership, medical directorship, or clinical program oversight

Responsibilities

  • Establish and maintain clinical standards aligned with hospice philosophy, CMS regulations, and best practices
  • Provide final authority on hospice eligibility determinations and recertifications
  • Oversee complex, high-acuity, and extended-stay patient cases
  • Ensure consistency in physician decision-making and documentation
  • Lead and manage hospice physicians and advanced practice providers
  • Standardize clinical documentation, performance expectations, and care delivery models
  • Drive accountability and engagement across contracted and employed providers
  • Serve as physician lead for survey readiness, audits, and compliance initiatives
  • Partner in QAPI programs and continuous quality improvement efforts
  • Ensure documentation supports medical necessity and audit defensibility
  • Provide leadership within interdisciplinary team structures
  • Collaborate with executive leadership on strategic initiatives, growth, and operational alignment
  • Serve as a clinical liaison to community physicians, hospitals, and referral partners
  • Guide appropriate utilization of hospice resources including medications and DME
  • Support financial sustainability while maintaining uncompromised patient care standards
