About The Position

ANX Hospice Care: ANX also provides 24/7 hospice care for individuals with limited life expectancies or for patients no longer willing to seek aggressive medical treatment. Hospice care is provided by our specially-trained team of health care professionals, including a medical director/physician, medical social worker, skilled nurse, home health aide, spiritual counselor and hospice volunteer. Requirements

  • Current CA Licensed Vocational Nurse (LVN) license.
  • Current CPR card for Healthcare Providers.
  • 1 year of Licensed Vocational Nurse (LVN) hospice experience.
  • Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
  • Demonstrates excellent verbal, written communication, and organization skills.

Responsibilities

  • Provide direct patient care and treatment in a home setting under the supervision of a Registered Nurse.
  • Ensure high quality patient experience and positive health outcomes by delivering the plan of care established by the RN Case Manager.
  • Prepare complete and timely clinical notes.
  • Coordinate care delivery with an interdisciplinary team (PT, OT, ST, MSW, and HHA).
