About The Position

Big Bend Hospice (BBH) is your “Hometown Hospice” providing compassionate end-of-life care for over 40 years to the Big Bend region. Guided by our values of integrity, respect, and excellence, we are seeking a dedicated LPN to join our East Home Care Team. The ideal candidate will work under the direction of the hospice Registered Nurse (RN), providing care to ill, injured, or convalescing patients or persons with disabilities in hospitals, nursing homes, clinics, private homes, group homes, and similar institutions. Position Duties and Responsibilities: The LPN for our East Home care team responsibilities will include, but are not limited to the following: Under the direction of the registered nurse, provides end-of-life care to patients and families. Monitor and evaluate medication needs and administration of patients, including symptom management, supplies, and equipment needs. Documents patient visits. Communicates progress and needs of patients and families to other interdisciplinary team (IDT) members promptly and professionally. Assess patient safety and provide education on safety issues to patients and families. Refer patients to other disciplines as needed. Educate patients and families on using durable medical equipment (DME) appropriately. Assist HHA in providing HHA services as needed. Provide RN Case Manager and triage with daily reports of patient progress. Deliver supplies for RN Case Manager as needed. Assists RN Case Manager in preparation of IDT review during team meetings and via reports. Provide HHA services in the absence of HHA while on-call during visits. Satisfactorily complete competency requirements for this position. Performs other job duties as requested.

Requirements

  • Minimum of one [1] year experience in a medical/surgical inpatient unit
  • Ability to physically access any home in the service area, i.e., navigate stairs, narrow spaces, lack of air conditioning and heat, etc.
  • Ability to remain calm and troubleshoot/handle patient/family crisis situations
  • Highly organized, with the ability to manage multiple priorities simultaneously and effectively.
  • Excellent verbal and written communication skills
  • Excellent organizational skills and attention to detail
  • Ability to act with integrity, professionalism, and confidentiality.
  • Ability to make decisions based on the expectations of supervisors.
  • Proficient with Microsoft Office Suite or related software including an EMR system.
  • Valid Florida driver’s license, auto insurance, and reliable transportation.
