This position focuses on following individualized Plans of Care, administering treatments, and partnering with the interdisciplinary team to promote comfort and support each patient’s quality of life. Requirements

• Holds current Licensed Practical Nurse (LPN) license in state(s) of practice.
• Graduate of an accredited practical nurse or vocational nursing program required.
• Ability to pass background, consumer report and drug test.
• Must have reliable transportation, valid driver’s license, and proof of auto insurance

Responsibilities

• Provide direct patient care, follow the Plan of Care, and support end-of-life needs.
• Document all care accurately and promptly while educating patients and caregivers.
• Coordinate care by communicating patient needs to the interdisciplinary team.
• Manage pain and symptoms, use equipment safely, and maintain full HIPAA compliance.
