Job Description Full-time Hospice LPN position Schedule: Monday thru Friday, 8a-5p Available territory openings: LPN to cover patients in 1-2 counties surrounding office Provides skilled and palliative nursing in the home as ordered on the plan of treatment and nursing care plan. Evaluates the significance of clinical data findings and communicates any pertinent information about the patient’s clinical status and ongoing needs to others in a timely fashion. Carries out the pain management and symptom management plan to keep the patient as comfortable as possible. Ability to adequately evaluate and re-evaluate pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Notifies the RNCM when pain is not manageable on the current medications. Demonstrates knowledge of medications and their correct administration based on age of the patient and patient’s clinical position. Benefits

Requirements

  • Licensed Practical Nurse in State of Indiana
  • Proof of current classroom CPR certification
  • Hepatitis profile and TB vaccination record
  • Current driver's license
  • Must have reliable transportation and insurance
