Requirements

• LPN license in the state of Pennsylvania.
• Graduate from an approved school of practical nursing.
• Two years of LPN experience, preferably in hospice.
• Current driver’s license and ability to spend approximately 20% of the day driving.
• Demonstrated experience with tablets, mobile phones, and EMR software.

Responsibilities

• Collaborate with the care team to manage and deliver patient care plans.
• Provide nursing care to hospice patients in Myerstown, PA and surrounding areas.
• Meet critical deadlines for charting and documentation.
• Self-manage time and schedule effectively.
• Drive to/from patient locations as needed.
