JOB SUMMARY: The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. The LPN works as a member of the Hospice team in providing Hospice care.

Requirements

  • Educational/Degree: Graduate from an accredited school of nursing
  • Currently licensed as an LPN to practice in the state of Arizona.
  • Ability to work independently, make accurate and timely judgments.
  • Ability to supervise others appropriately.
  • Ability to respond appropriately to crisis outside of a hospital setting.
  • Acceptance of and adaptability to all social, racial, cultural and religious backgrounds.
  • This is a mobile position. Candidate must have a current valid driver’s license, auto liability insurance and reliable transportation.

  • Training/Licensure: Currently licensed as an LPN to practice in the state of Arizona.

Responsibilities

  • Provides direct patient care as defined in State Nurse Practice Act.
  • Implements current nursing practice following a patient assessment and an approved plan of care initiated by the Registered Nurse.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Teaches, supervises and counsels the Hospice patient and family members about providing care for the patient.
  • Observes signs and symptoms and reports to the physician and IDG members any unexpected changes in the patient’s physical or emotional condition.
  • Uses equipment and supplies effectively and efficiently.
  • Provides appropriate pain/symptom management. Evaluates patient’s response to treatments/medications.
  • Knowledge of state, federal, local and accreditation (Joint Commission) regulations for the delivery of hospice services.
  • Participates in personal, professional growth and development. Also participates in organization’s quality management program.
  • Meets daily average productivity standards set by the agency.
  • Performs other duties as assigned by the Case Manager and/or Supervisor.
  • Assists with training other nursing service personnel.
  • Performs specific nursing procedures as needed (e.g., treatments, management of symptoms) following doctor’s orders.
  • Attends team conferences as requested.
  • Maintains records as required by Hospice.
  • Follows the policies and procedures of Hospice. Observes confidentiality and safeguards all patient-related information in compliance with HIPAA regulations.
  • Always communicates to the supervisor if unable to meet a patient’s need or perform a procedure.
  • Participates in on-call system and is responsible for providing on-call coverage when unavailable for assigned duties.
  • Maintain skills and knowledge.
  • Works with interdisciplinary group concept of patient care.
  • Coordinates the implementation of the plan of care for patients residing in SNF, NF, ICF or MR.
  • Organizes work schedule and utilizes time management to be able to attend all required meetings.
  • Complies with agency infection control policies and protocols.
