Requirements

Associate degree, higher education is valued.
Substantial experience of over 2 years in Hospice Care.
A minimum of a year's immersion in using Electronic Medical Record (EMR), or similar EMR, or TruChart EHR.
A valid Licensed Practical Nurse (LPN) certification/license authorized for Pennsylvania (PA).
A duly acknowledged Regular Driver License (DL) for mobility purposes.

Responsibilities

Collaboratively overseeing and executing the management and delivery of patient care plans.
Rendering care and assistance to patients in Penn Hills and its vicinity.
Ensuring the timely meeting of crucial charting deadlines.
Effectively managing individual work hours and schedules.
Exhibiting adeptness with tablets, mobile phones, and EMR software usage.
