About The Position

Job Description: What You’ll Do: Perform basic nursing assistant procedures as indicated in competency assessment to include but not limited to taking and recording of vital signs, simple, non-sterile dressing changes, bed bath, sponge tub and shower bath, shampooing, nail and skin care, oral hygiene, toileting and elimination, nutrition and fluid intake, application of ointments, creams and topicals, reporting on skin conditions within State of New Hampshire LNA Scope of Practice. More Specifically: • Provides services that are ordered by the physician included in the plan of care, permitted to be performed under state law and consistent with home health training. Performs tasks and provides personal care to patients following the plan of care in a safe and effective manner complying with infection prevention and control policies. Monitors condition of patient and reports changes to appropriate personnel. • Performs transfers, range of motion, positioning, exercises, and assistance with activities of daily living ADLs, ambulation, functional activities and other basic nursing care as instructed. • Completes all documentation in the EMR and in accordance with the agency timeliness policies. Participates in the weekend/holiday rotation as defined by Beth Israel Lahey Health at Home. • Participates in in-service programs What You’ll Need: • High School Diploma or equivalent • Licensed Nursing Assistant - N.H. state • Valid Driver’s License • CPR certified or within 30 days of hire • One year experience as a Hospice Aide preferred. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law. Requirements

  • High School Diploma or equivalent
  • Licensed Nursing Assistant - N.H. state
  • Valid Driver’s License
  • CPR certified or within 30 days of hire
