About The Position

Concordia Hospice of Washington is a partnership between Concordia Community Support Services and Washington Health System that offers both quality home care and inpatient hospice end-of-life care. Concordia Hospice of Washington serves our patients in Washington and Greene counties with the focus to provide services that offer comfort, peace, and dignity for those with incurable illnesses and for their families. Concordia Hospice of Washington is seeking a compassionate and dependable On-Call Licensed Practical Nurse (LPN) to provide comfort-focused care and support to patients and families outside of regular business hours. This role is ideal for a nurse who is confident working independently and committed to delivering meaningful, patient-centered care. This position requires travel throughout the South Hills region and Washington County. Requirements

A Licensed Practical Nurse with a current, active license
Calm, confident, and comfortable working autonomously
Compassionate and attentive to patient and family needs
A clear and thoughtful communicator
Organized, dependable, and responsive
Comfortable traveling within assigned territories

Responsibilities

Respond to patient needs during evenings, nights, and weekends
Provide skilled nursing care focused on comfort and symptom management
Assess patient conditions and communicate updates to the care team
Offer education, reassurance, and guidance to patients and families
Deliver care with compassion, professionalism, and sensitivity
Represent Concordia Hospice of Washington with empathy and respect
