Requirements
- Current Michigan LPN License in good standing.
- Minimum of 1 year experience in hospice or palliative care.
- Strong empathy and communication skills.
- Flexibility to respond to varying patient needs.
Responsibilities
- Conduct initial and ongoing assessments of patient needs.
- Implement and adjust individualized care plans.
- Ensure compliance with hospice policies and Medicare regulations.
- Educate patients and families on self-care and end-of-life options.
- Supervise paraprofessionals delivering patient care.
- Document and report clinical findings and participate in interdisciplinary meetings.
- Enhance patient satisfaction and experience.
