Position Summary: The Licensed Practical Nurse, reporting directly to the Director of Clinical Services, is responsible for providing high-quality nursing care in the home setting under the direction of a Registered Nurse. This role supports the physical, personal, and emotional well-being of patients, assists in care planning and evaluation, and promotes the goal of helping each patient reach their maximum level of self-care and independence, when possible.

Requirements

  • Active LPN license in the state of practice.
  • One (1) or more years of hospice experience, required.
  • Valid driver’s license and reliable transportation, required.
  • CPR certification, required.
  • Strong clinical skills including medication administration and wound care, required.

Responsibilities

  • Provide direct skilled nursing care to hospice patients in accordance with the plan of care.
  • Monitor patient condition, including vital signs, symptoms, and pain levels, and report changes to the RN.
  • Administer medications, treatments, and wound care as ordered by the physician.
  • Support patients with activities of daily living (ADLs) such as bathing, dressing, and mobility.
  • Assist with symptom management and comfort-focused care for end-of-life patients.
  • Educate patients and families on disease progression, medications, and care techniques.
  • Document all patient care, observations, and interventions accurately and timely.
  • Collaborate with the interdisciplinary team (RN, social worker, chaplain, therapists) to ensure coordinated care.
  • Support RN case managers by performing routine visits, follow-ups, and stable patient check-ins.
  • Assist with admissions, discharges, and transitions of care as needed.
  • Other duties as assigned.
