About The Position
University Home Care Inc is seeking a dedicated Hospice Licensed Practical Nurse (LPN) for an in-office position serving Livonia and Royal Oak, MI. This role involves providing compassionate hospice care to patients, ensuring comfort and quality of life in their homes and in-office settings.
Requirements
- Valid LPN license in the state of Michigan.
- Previous hospice or palliative care experience preferred.
- Strong communication and interpersonal skills.
- Ability to work both independently and as part of a multidisciplinary team.
- Reliable transportation for potential travel between Livonia and Royal Oak offices.
- Basic computer proficiency for documentation purposes.
Responsibilities
- Provide direct nursing care to hospice patients according to care plans.
- Collaborate with interdisciplinary team members to coordinate patient care.
- Monitor and report changes in patient condition to the healthcare team.
- Educate patients and families on care techniques and symptom management.
- Complete and maintain accurate clinical documentation.
- Participate in patient and family meetings and discharge planning.
- Assist with in-office patient visits and support nursing staff in Livonia and Royal Oak locations.
