About The Position

To be an educational resource for healthcare professionals, to seek out marketing opportunities for Hospice El Paso, to maintain positive relationships with clients, potential referral sources with a focus to build census, increase market share and to improve community knowledge of Hospice services. The Hospice Liaison is primarily engaged in establishing and building relationships with professional providers in the El Paso community and is responsible for working within the healthcare community to: assure that professionals are aware of the scope of Hospice services and have an understanding of admission criteria; have knowledge of an information on community resources available to non-funded patients; effectively present the Hospice program of care; and assist in assuring that open and effective communication channels exist to facilitate the provision of the best possible in-home palliative services for patients and their families.

Requirements

  • High School Graduate, Bachelor’ Degree desirable – may consider equivalent experience.
  • Prior experience in customer services, sales or marketing, preferably in a medical environment.
  • Excellent interpersonal and communication skills.
  • Excellent written and oral presentation skills.
  • Organized and detail oriented.
  • Manages time effectively, meets deadlines.
  • Must be able to read, write, and speak English at a level that will permit the employee to accurately understand and communicate information as needed to safely and efficiently perform job duties
  • Persistent and goal oriented.
  • Personal vehicle available for transportation during work hours and personal phone.
  • Valid driver’s license and auto insurance as required by the state of Texas.
  • Physically fit and able to work in a variety of settings and locations, primarily in well lighted/ventilated areas.
  • Able to cope with the mental and emotional stress of the position.
  • Sits, bends, lifts, and moves intermittently during working hours.
  • Lifts and move items, generally not to exceed 20lbs.
  • Possesses sight/hearing senses or uses prosthetics that enable these senses to function adequately to meet the requirements of the position.

Responsibilities

  • Executes and follows a written plan designed in cooperation with the Operations Officer for contacting healthcare professionals to share information (educate the staff and patients) about Hospice availability and the benefits of Hospice services.
  • Contacts assigned physicians and facilities in a systematic manner through frequent short visits to discuss patients and the applicability of Hospice services to meet their needs.
  • Meets weekly, and as appropriate, with the Hospice team to obtain information for use in communication with referring professionals and their staff.
  • Maintains a daily log of contacts, activities and the nature of discussions.
  • Prepares weekly reports for the Operations Officer demonstrating the number and quality of contacts.
  • Maintains a mailing list of professionals for newsletters, greeting cards, invitations to Hospice functions, etc.
  • Makes presentations to groups in the medical, educational and general communities (example: in-services with a registered nursing team leader or social workers for nursing homes).
  • Coordinates requests for in-service educational meetings with the Operations Officer.
  • May perform the following patient support services: Introduces Hospice care to the patient/family at a contracted hospital or nursing facility Obtains initial paperwork from the patient/family Assists the staff at the hospital/nursing facility to expedite the care process by making copies of specified information and delivering this to Hospice.
  • Works directly with providers to identify and solve problems. Refers difficult or complicated problems identified in the professional community to the Operations Officer.
  • Facilitates the exchange of information and the flow of paperwork between the Hospice office and practitioners and facilities in assigned territory.
  • Works with the Operations Officer in the planning and implementation process to enhance agency marketing goals, market penetration and the development of marketing materials which best represent Hospice El Paso.
  • Attends and participates in intra-agency and other professional meetings and conferences (example: professional education seminars/events; assist with the Black, Hispanic, and Greater Chamber of Commerce events; assist with the Medical Advisory meeting each quarter and Symposium) to network and develop relationships which will benefit Hospice El Paso.
  • Functions independently, demonstrates personal integrity, flexibility and the ability to work effectively with other personnel under varying circumstances.
  • Has knowledge of and follows Hospice policies and procedures.

  • Establishes and maintains effective communication and a cooperative manner with patient/family members, physicians, professionals and support staff working in hospitals, nursing facilities, foster homes, and other related facilities as required by position.

  • Communicate effectively, both verbally and in writing, with all levels of personnel in the Agency.
  • Complies with HIPAA standards by abiding with minimum Protected Health Information necessary to perform job description responsibilities. Access level 2 – Select, insert, update.
  • Attends and participates in in-service education classes and on-the-job training programs as scheduled.
  • Attends conferences and workshops as approved.
  • Involved in community/civic health matters/projects.
  • Attends ethics and compliance training and maintains compliance with the Corporate Compliance Program.
