Requirements

  • Possession of a current license to practice as a registered professional nurse or the equivalent license and education to practice as a healthcare professional in the practicing state
  • Minimum 3-5 years of nursing experience, with at least 2 years in hospice, palliative care, or related specialty
  • Previous supervisory or charge nurse experience in healthcare, homecare or hospice
  • Strong knowledge of end-of-life care practices and symptom management
  • Demonstrated experience with electronic medical records, HCHB experience strongly preferred
  • Proficiency with Microsoft Office Suite and financial software systems
  • Excellent organizational and time management skills with ability to manage multiple priorities
  • Strong analytical and problem-solving abilities
  • Exceptional attention to detail and accuracy in documentation
  • Excellent communication and interpersonal skills
  • Ability to work independently and make sound clinical and operational decisions
  • Valid drivers license and proof of insurance is required
  • Must possess current CPR Certification

  • HCHB experience strongly preferred

Responsibilities

  • Oversee EMR functions, including SOC, POC, CTIs, HOPE documentation, and management coordination notes.
  • Review and approve care plan changes, medication orders, and aide care plan updates.
  • Process HEAs and ensure accurate location settings.
  • Collaborate with Quality and CTAs to remove holds.
  • Complete weekly RN notes for IDG meetings.
  • Review charts for GIP appropriateness and coordinate with social workers, field staff, and the medical director.
  • Document comprehensive hospice narratives for all direct admissions.
  • Develop and maintain daily/weekly schedules for nursing, CNA, physician, and support staff.
  • Manage call-ins, shift trades, agency staffing, and weekend/MD coverage.
  • Participate in hiring processes, orientation, annual reviews, competencies, TB testing, and tracer documentation.
  • Order, organize, and reconcile supplies across multiple vendors.
  • Complete ACHC audit tasks.
  • Collaborate with Facilities team on oxygen supply issues, porter services, and maintenance requests.
  • Performs other duties as assigned
