The Hospice IPU RN is responsible for facilitating the transition of patients from hospital care to hospice services, ensuring that they receive comprehensive and compassionate end of life care. This role involves coordinating with healthcare providers, patients and families to provide a seamless transition and support throughout the hospice care process. This position will work on-site within a hospital located in Atlanta/Sandy Springs area working 7:00am -7:00pm; 7 days on / 7 days off. The RN will manage and oversee the care plan and provide rounds daily of the patient housed in the hospital. Why work for Capstone Hospice?
