JOB SUMMARY As a Hospice Intake Coordinator at Namaste Health, you will play a vital role in coordinating and ensuring the seamless intake process for our patients. Your responsibilities will include managing the admission process to ensure a smooth onboarding for patients, collaborating with our interdisciplinary team, obtaining necessary documentation, and ensuring all regulatory guidelines are followed. Your compassionate nature, attention to detail, and strong communication skills will be instrumental in providing the best possible care for our patients and their families. II. DUTIES & RESPONSIBILITIES 1. Assists and participates in all daily patient referral and intake operations including clinical assessment. 2. Ensures compliance with all state and federal referral/intake regulatory requirements. 3. Directs the implementation of improved work methods and procedures to ensure patients are admitted in accordance with policy. 4. Establishes and maintains positive working relationships with current and potential referral sources. 5. Ensures consistent and timely transition of patients to home or hospice care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers. 6. Builds and monitors community and customer perceptions of Namaste Health as a high quality provider of services. 7. Gathers, collates, and reports referral statistics including key customer referral trends. 8. Participates in strategic planning including identifying opportunities for additional or improved services to meet unmet customer needs. 9. Maintains comprehensive working knowledge of Namaste Health’s contractual relationships and ensures that patients are admitted according to contract provisions. 10. Participates in quality assessment performance improvement teams and activities. 11. Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Namaste Health. 12. Leads Hospice-portion of Morning All-Staff call; prepares necessary reports and information to appropriate staff; provides necessary info regarding admissions for the day; participates in Weekend Friday prep call, and provides pertinent intake information 13. Checks Fax and Referral portals to monitor for referrals 14. Completes all necessary Hospice Intake and Scheduling workflow 15. Participates in Triple Check process (Intake portion) 16.
