About The Position

Responsible for coordinating/assuring a high standard of Hospice patient care consistent with each profession on the interdisciplinary team. Supports the Hospice House Manager in assuring continuous, quality, cost-effective operations. Promotes positive teamwork focused on delivering a high standard of care.

Requirements

  • Graduate of an accredited school of nursing with an unencumbered licensure to practice professional nursing in the State of Oregon.
  • Excellent assessment, problem-solving skills, and nursing skills per the competency checklist.
  • Strong clinical judgment and patient triaging skills are vital; efficient time management with minimal oversight; communication skills (with other care team members, including physicians); positive phone interaction/customer service; and computer skills.
  • One to two years of recent medical/surgical care experience in an institutional setting.
  • Ability to pass a background test, drug screen, and TB test.
  • Current Food Handler’s certification required.
  • Valid CPR Certification.
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Compassion, empathy, and dedication to patient care.

Nice To Haves

  • 2 years of supervisory experience is preferred.

Responsibilities

  • Adheres to all policies, procedures, and regulations regarding patient care, safety, fire, security, and risk management.
  • Provides for a safe environment for patients, guests, visitors, and employees.
  • Provides leadership, problem-solving, and decision-making in management.
  • Facilitates communications and works effectively between Facility staff, patients/families/caregivers, physicians, vendors, and other departments and professionals as appropriate.
  • Serves as a liaison with other departments to resolve problems and improve efficiency and effectiveness.
  • RN/LPN/CNA Scheduling – Plans/schedules nursing staffing in collaboration with Hospice House Manager to meet the needs of the Facility per ORS 441.155 and OAR 333, Division 510 regulations.
  • Approves/schedules RN/LPN/CNA time off requests.
  • Arranges alternate staff coverage in response to patient acuity and unscheduled staff absences.
  • Reviews time sheets.
  • Communicates staffing and scheduling concerns with the Hospice House Manager.
  • Actively participates in staff meetings, department meetings, management meetings, in-services, competency training, and other related activities.
  • Evaluates the quality of care delivered by professional staff, as evidenced by clinical assessments and medical record review.
  • Monitors documentation and implementation of physical, psychosocial, and spiritual interventions for the patient/family/caregiver.
  • Provides leadership and formal/informal education to staff through in-services, case conferences, and group discussions on Hospice philosophy, operations, policy, and procedures, and problem-solving.
  • Articulates the needs of patients transferring between levels of care to staff and promotes continuity of care.
  • Provides direct patient care up to 25% of working hours.
  • Ensure IDT notes and orders are signed in a timely manner.
  • Assists with employee hiring decisions. Serves as orientation preceptor; provides ongoing education to patient care staff.
  • Supervises facility staff as assigned; Completes timely employee evaluations.
  • Rotates after-hours calls for Hospice House. May be called upon to cover shifts and participate in various duties related to patient care.
  • Actively participates in quality assurance, utilization review, and risk management activities.
  • Facilitates patient care and quality management activities by assuring staff participation and Facility representation on focus groups; follows up on surveys and grievances; performs medical record and utilization review audits.
