Requirements

  • A Diploma or associate degree in nursing is required; Bachelor’s Degree preferred.
  • A current, active, and unrestricted compact (multistate) license as a Registered Nurse (RN) is required.
  • Current CPR required; CHPN preferred.
  • Minimum 1-year related experience for inpatient units (IPU) and 2 years for other nursing positions.
  • Experience in direct care of patients with life limiting illnesses required.
  • Computer proficiency is required.

Responsibilities

  • Delivery of Patient Care
  • Assumes responsibility for the coordination and delivery of patient/caregiver care.
  • Conducts nursing assessments to assist in the overall management of patient/caregiver needs.
  • Develops, implements, evaluates, and updates the plan of care to ensure that care is uninterrupted and that goals are achieved.
  • Facilitates patient and caregiver independence to the extent possible.
  • Utilizes VIA Health Partners patient related protocols, standing orders and guidelines when providing hospice care.
  • Conducts effective patient and caregiver education.
  • Encourages and allows time for questions and consultation.
  • Communicates effectively with members of the IDG, patients, caregivers, and other health professionals to ensure continuity of care.
  • Maintains open lines of communication with supervisor.
  • Applies principles of infection control and follows guidelines for universal precautions.
  • Attends, participates, and may facilitate interdisciplinary group conferences [IDG] and special care conferences.
  • Establishes and maintains effective working relationships with team members.
  • Ensures accurate, complete, succinct, and timely clinical documentation.
  • Demonstrates competency in knowledge and skills.
  • Maintains current professional licensure, CPR certification and CEU requirements.
  • Exhibits time management skills.
