Requirements

• High School diploma preferred, plus training/certification in related field required.
• Certified Nursing Assistant; maintain current listing status required.
• Minimum 1 year experience required.
• Experience in home care, working with terminally ill and/or geriatrics preferred.

Responsibilities

• Delivery of patient care
• Provide personal care (ADL) including bathing, body mechanics, nutrition, elimination, safety, and special procedures.
• Perform housekeeping services necessary for the health and safety of the patient/family/caregiver.
• Participate in interdisciplinary team conferences (IDT) and special case conferences.
• Ensure accurate, complete, succinct, and timely clinical documentation.
• Demonstrate competency in knowledge and skills.
• Assume responsibility for professional development and staying abreast of current trends in the healthcare field.
• Maintain productive and professional relationships with external agencies and healthcare providers.
