About The Position

The Hospital Liaison RN at Affinity Care of Virginia is responsible for promoting continuity of care between medical facilities, physicians, and patients in hospice and palliative care settings. This role involves educating patients and families about hospice services, assessing patient needs, and ensuring appropriate care delivery in compliance with Medicare guidelines. The position requires strong communication skills and the ability to establish positive relationships with referral sources and healthcare teams.

Requirements

• Registered Nurse with a valid RN license in Virginia.
• Minimum of one year experience in an acute care setting or as a case manager.
• Bachelor's degree in nursing preferred.
• Basic math skills and strong communication skills.
• Experience in hospice, home care, or public health nursing is preferred.

Nice To Haves

• Supervisory experience is a plus, though not required.

Responsibilities

• Establish positive working relationships with referral sources, medical center staff, and patients.
• Educate patients, families, and staff about hospice and palliative care services.
• Coordinate agency referrals and assess patients for admission criteria.
• Participate in hospice activities and maintain productivity standards.
• Assist patients and families in making informed decisions regarding care options.
• Develop and initiate care plans in collaboration with patients and families.
• Document patient assessments and care plans accurately and timely.
• Communicate effectively with physicians and community health personnel regarding patient needs.
