About The Position

The Community Hospice has a wonderful opportunity for a Per Diem LPN to work as a member of our Hospice Home Care Team caring for patients and families in both the homecare and nursing home settings. Primary region to cover is Saratoga and Washington Counties. What you will do: Coordinate closely with our RN Supervisor and RNs to provide nursing and personal care services as a part of a comprehensive plan of care to provide added support to patients and families This is an ideal opportunity for someone who enjoys working closely with a team (our teams include RNs, Aides, medical directors with specialized expertise in end of life care, social workers, chaplains, complementary therapists and volunteers). Training provided in end of life care.

Requirements

Valid NYS LPN License
Minimum 1 year LPN experience required
Strong computer skills
Organization and communication skills and ability to work as part of a team

Nice To Haves

Prior hospice/ home care experience preferred, but not required
