About The Position

Agrace is a nationally recognized nonprofit leader in hospice and palliative care, seeking a skilled CNA to provide meaningful, patient-centered care. The CNA Homecare (HC) is responsible for ensuring patient and family satisfaction by assisting with activities of daily living, providing light homemaker help, following the plan of care, communicating changes in condition, and documenting care. This role provides patient and family centered care consistent with Agrace's philosophy, goals, standards, regulations, and policies. For over 45 years, Agrace has been an innovator in patient and family care, and in educating others about death, dying, and grief, serving as the largest hospice provider in Wisconsin. Agrace also supports people across southern Wisconsin who are aging, seriously ill, or grieving. Requirements

• High school diploma or GED, or equivalent combination of education and work experience
• Graduation from an accredited Certified Nursing Assistant program
• Current, unrestricted Nursing Assistant Certification in state of employment
• Valid driver’s license, auto insurance, and access to a reliable vehicle
• Basic Microsoft Office skills
• Experience using electronic health records
• Ability to maintain confidentiality about patients, families, caregivers, staff, donors, and organizational intellectual property
• Ability to use proper body mechanics, safe patient handling, and standard precautions

Nice To Haves

• One-year personal care experience, hospice, or home health, preferred

Responsibilities

• Assist with ambulation, changing position, and moving to/from sitting and standing
• Help with bathroom functions like toileting and incontinence care
• Help bathe, shower, dress, provide oral care, and groom
• Assist with eating and drinking
• Perform post mortem care
• Obtain vital signs
• Assist with treatments
• Anticipate and respond to patient needs
• Perform light housekeeping such as changing linens, tidying room, doing laundry, and removing trash
• Order and deliver patient supplies
• Help prepare simple meals
• Read, understand, and follow the plan of care
• Document cares and changes in patient condition in electronic health record
• Document and report significant change in condition or unusual incidents to appropriate Interdisciplinary Team Member such as client abuse/neglect, suicidal intent, harmful coping mechanisms or violence, etc.
• Reinforce patient, family, and facility partner education
• Participate in meetings to plan and evaluate patient/family services
• Precept new staff and students
• Delegate tasks to volunteers as appropriate
