About The Position

The Hospice Home Team RN Case Manager plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses wherever they call home. This is a full-time, salaried position working a flexible Monday - Friday 8-4:30pm schedule with a minimal on-call requirement. The primarily territory for this role is Sheboygan County, but we also serve patients in Manitowoc & Ozaukee Counties. This position is responsible for coordinating and managing comprehensive hospice care plans that address the physical, emotional, and psychosocial needs of patients and their families. The RN Case Manager collaborates closely with interdisciplinary teams, including physicians, social workers, and chaplains, to ensure seamless communication and continuity of care. By conducting thorough assessments and monitoring patient progress, the RN Case Manager ensures that care is both effective and aligned with the patient’s goals and wishes. Ultimately, this role aims to enhance the quality of life for patients while supporting their families through the hospice journey with dignity and respect.

Requirements

  • Current and valid Registered Nurse (RN) license in the state of practice within the United States.
  • Minimum of two years of clinical nursing experience, preferably in hospice, palliative care, or home health settings.
  • Ability to perform comprehensive patient assessments and develop individualized care plans.
  • Proficiency in electronic medical records (EMR) documentation and basic computer skills.
  • Certification in Hospice and Palliative Nursing (CHPN) within 3 years.

Nice To Haves

  • Strong knowledge of hospice care principles, symptom management, and end-of-life care a plus.

Responsibilities

  • Conduct comprehensive patient assessments to develop individualized hospice care plans.
  • Coordinate and oversee the delivery of nursing care and symptom management in the home setting.
  • Collaborate with interdisciplinary team members to update and adjust care plans based on patient needs.
  • Provide education and support to patients and families regarding disease progression, symptom control, and available resources.
  • Document all patient interactions, care plans, and clinical updates accurately and timely in compliance with regulatory standards.
  • Serve as a liaison between patients, families, healthcare providers, and community resources to facilitate holistic care.
  • Respond promptly to changes in patient condition and coordinate appropriate interventions or referrals.
  • Ensure compliance with hospice policies, procedures, and regulatory requirements.
