About The Position

Duties/Responsibilities: Provides patient care, ensuring patient safety and comfort, according to the Home Health Aide assignment form. Reports and records any changes in condition or behavior of the patient. Adapts to changes in workload or patient assignments without interference to patient care. Responsible for completion of assignments, maintaining quality work. Establishes good rapport with patients and families, respecting patients rights to privacy and confidentiality of information. Accurately communicates in writing through utilization of appropriate charting methods, and turns in all paperwork on time. Provides support to patients and families in a courteous manner. Performs routine duties without physical supervision. Accurately communicates patients information to the RN Case Manager, LPN and/or Assistant Director of Clinical Services. Assists in maintaining equipment assigned to patient care, and reports malfunctioning equipment to appropriate personnel. Any and all other duties and responsibilities as assigned. #HOSPICE

Proficient with Microsoft Office Suite or related software.
High School Diploma, or GED equivalent.
Current Home Health Aide certification.
Valid CPR Certification.
Certified to pass Home Health Aide Competency Test.
Ability to speak, hear, and read English.
Lift up to 20 lbs. from floor to 36, and from 36 to 64.
Sitting.
Walk up to .25 miles.
Stand.
Sustained forward bending/sitting.
Repetitive Squat.
Push/Pull up to 75 lbs. of force at 36.
Kneel.
Full body range of motion.
Grasp of various sizes and weights up to 50 lbs.
Sustained squat.
Balance in walking, standing, carrying, and moving patients and equipment, etc.

Nice To Haves

1 year of Home Health Aide or LTCF experience preferred.
