About The Position

Suncrest Hospice is seeking caring and compassionate Hospice Aides to join our team! This position will travel throughout our territory, providing care for a group of hospice patients where they reside, which could include private homes and skilled nursing facilities. At Suncrest Hospice, our goal is to change the expectation of hospice care in your area by providing exceptional care and service to our patients. This is achieved by allocating the resources to increase our staff-to-patient ratios, thereby increasing clinical visits while lowering clinician caseloads. Requirements

• Strong commitment to improving quality of life while partnering with community organizations in patient care.
• Ability to work well within a team environment to collaborate daily with nurses, social workers, chaplains, volunteers, and HHA leadership.
• Must be willing to drive with reliable transportation, valid driver’s license, and auto insurance.
• HHA certification in the state(s) of New Jersey required.
• CPR certified.

• Ability to work independently and on your own on a day-to-day basis.

Responsibilities

• Provide resident care to assigned residents in accordance with resident assessments and care plans.
• Work under the direction of a Case Manager.
• Coordinate the proper types of care to residents with other licensed care staff.
• Follow all Company nursing policies and procedures as well as safety guidelines.
