About The Position
University Home Care Inc is seeking a compassionate and dedicated Hospice Home Care Registered Nurse to join our team in Livonia, MI. In this role, you will provide high-quality, patient-centered nursing care to hospice patients in their homes, ensuring comfort and dignity during end-of-life care.
Requirements
- Current and valid Registered Nurse (RN) license in the state of Michigan.
- Experience in hospice, home health, or palliative care preferred.
- Strong interpersonal and communication skills.
- Ability to work independently and manage patient caseload effectively.
- Compassionate with a patient-focused approach to care.
- BLS/CPR certification required.
Responsibilities
- Provide skilled nursing care to hospice patients in their home environment.
- Assess and monitor patient conditions, managing symptoms effectively to ensure patient comfort.
- Coordinate care with interdisciplinary team members, including physicians, social workers, and chaplains.
- Educate patients and their families about disease processes, symptom management, and end-of-life care options.
- Document patient care accurately and timely according to organizational policies and regulatory requirements.
- Respond promptly to patient and family needs, providing emotional support and guidance.
