About The Position

Join Life Care Home Health Family! Serving Texas, Florida, Nevada, Georgia, and Arizona, we provide skilled nursing, therapy, homecare, hospice, palliative care, and private duty services. The GIP RN for Infinity Hospice Care (IHC), works primarily in hospitals where IHC holds a current contract for hospital GIP and supports the hospital inpatient palliative care service. This GIP RN gains a high understanding of the IHC intake process and support transitions to both home IHC and NEVPC. The GIP RN promotes the hospice program, enrolls patients, and supports providers when needed to transition patients to IHC and NEVPC. The GIP RN engages patients, families, caregivers, and updates hospital staff as needed. The GIP RN acts as a liaison in supporting documentation, notes, and electronic information transmission to both IHC and NEVPC.

Requirements

  • Active nursing license from an accredited program and in good standing with the State Board of Nursing in the state of licensure
  • Strong hospice knowledge
  • Outgoing personality, team player and maintain confidentiality
  • Ability to prioritize and be flexible
  • Must maintain a valid driver’s license and current automobile insurance

Nice To Haves

  • Proven success in healthcare marketing preferred

Responsibilities

  • Can perform hospice admission for both hospital GIP and start the admission process for patients transitioning to home setting.
  • Ensures an excellent hospital to home hand off.
  • Works with social services and/or placement when most appropriate care setting is difficult to determine.
  • Utilize marketing material, discuss key programs and benefits of IHC and NEVPC with hospital patient and family
  • Use and promote referral screening forms and assist in presentations to case management, facility stakeholders, and families
  • Assist in gathering information to expedite hospital discharge to SNFs, help resolve issues and assist in placement.
  • Understands IHC inpatient admission criteria.
  • Ensure visit feedback and patient outcomes are commun
