About The Position

This is a full-time weekend nursing position for either a Registered Nurse (RN) or a Licensed Practical Nurse (LPN) specializing in hospice care, located in the Kingfisher area. The designated shift is from Friday 5:00 PM to Monday 8:00 AM, and the role requires significant travel, up to 100%, to patient homes or facilities. The position involves comprehensive patient care, including assessments, medication management, care plan updates, and direct nursing interventions, with a strong emphasis on communication with medical staff, patients, and families. Both RNs and LPNs are expected to participate in staff meetings, in-services, and new personnel orientation, and to perform duties with sound judgment and a courteous, positive demeanor.

  • Valid CPR Certification (for LPN).
  • 1 year of experience in home health, LTCF, or related field nursing (for LPN).

Responsibilities

  • Oversees patient cases from admission to death/discharge (RN).
  • Completes all nursing assessments for each patient assigned (RN).
  • Responsible for maintaining patient medication profiles, updating as needed, while ensuring updated profile is in patient charts at the office, as well as in the facility or home (RN).
  • Updates the plan of care for each assigned patient every two weeks (RN).
  • Completes the re-certification process by assessing and accurately documenting findings on appropriate forms (RN).
  • Makes RN supervisory visits every 14 days on all assigned patients (RN).
  • Completes admissions as assigned (RN).
  • Provides on-call hours as determined by Patient Care Manager (RN).
  • Accurately communicates patients’ information to Patient Care Manager and medical staff (RN).
  • Participates in sharing of knowledge through in-services, staff meetings, and assisting in orientation of new personnel (RN & LPN).
  • Performs routine duties without physical supervision (RN & LPN).
  • Recognizes and responds to priorities in the patient care process (RN).
  • Makes sound judgements based on factual information, while seeking guidance and direction as needed for performance of all duties (RN).
  • Provides support to patients and families in a courteous manner with a positive tone (RN & LPN).
  • Assesses, plans, implements, and evaluates patient care (RN).
  • Provides nursing care to patients according to established policies and procedures (RN & LPN).
  • Checks status of medications provided by Hospice at each visit, and reorders as needed (RN & LPN).
  • Takes and records vital signs of the patient in an accurate and precise manner (RN & LPN).
  • Reports and records any changes in condition or behavior of the patient (RN & LPN).
  • Communicates concerns and/or changes to all disciplines (RN).
  • Assists in the assessment, planning, implementation, and evaluation of patient care within the nursing care plan (LPN).
  • Uses proper techniques, procedures, and safety precautions to meet individual needs of patients (LPN).
  • Performs skilled nursing visits as scheduled (LPN).
  • As authorized, prepares for set-up, hangs, monitors, and discontinues intravenous fluids with accuracy according to direction (LPN).
  • Provides direct care to patients, ensuring safety and comfort of the patient (LPN).
  • Performs treatments and/or assists in treatments and procedures with attention to detail, accuracy, patient safety, and comfort (LPN).
  • Records all care provided, following established guidelines for charting (LPN).
  • Utilizes the nursing process, and is familiar with other duties, recognizing their importance to patient care (LPN).
  • Participates in patient and family teaching and instruction (LPN).
  • Responsible for completion of assignments while maintaining quality work (LPN).
  • Able to adapt to changes in workload or patient assignments without interference to patient care (LPN).
  • Able to perform in an accurate and precise manner in emergency situations when time is of the essence (LPN).
  • Establishes good rapport with patients, families, and visitors, respecting patients rights to privacy and confidentiality of information (LPN).
  • Communicates pertinent information regarding patient to Interdisciplinary Team, using nursing care plan approach (LPN).
  • Independently performs assigned duties, including after-hours calls (LPN).
  • Makes sound judgments based on factual information, seeking guidance and direction, when necessary, for performance of duties (LPN).
  • Assist in maintaining equipment assigned to patient care, and reports malfunctioning of equipment to appropriate personnel (LPN).
