Summary: We are seeking an experienced leader to oversee and guide an interdisciplinary team of nurses, home health aides, social workers, and chaplains serving the territory south of I-10. This role provides direct leadership and supervision, fostering a culture of collaboration, accountability, and excellence in patient-centered care. The ideal candidate will strategically coordinate and direct team activities to ensure efficient operations, strong team engagement, and high-quality outcomes. Responsibilities include ensuring consistent communication and care updates, upholding policies and procedures, and managing team performance and resources within budgetary guidelines.
