This position is recruiting for an RN Team Leader to oversee care services in the territory south of I-10. Summary: We are seeking an experienced leader to oversee and guide an interdisciplinary team of nurses, home health aides, social workers, and chaplains serving the territory south of I-10. This role provides direct leadership and supervision, fostering a culture of collaboration, accountability, and excellence in patient-centered care. The ideal candidate will strategically coordinate and direct team activities to ensure efficient operations, strong team engagement, and high-quality outcomes. Responsibilities include ensuring consistent communication and care updates, upholding policies and procedures, and managing team performance and resources within budgetary guidelines.

Requirements

  • Graduate of an accredited school of nursing with current RN license to practice in the state of Texas
  • Home health, hospice or public health nursing experience
  • Ability to supervise staff and coordinate services
  • Five years minimum management experience, hospice/palliative experience preferred.
  • Excellent organizational, administrative, interpersonal and communication skills
  • Ability to multi-task, effectively
  • Ability to accommodate multi-faith beliefs and backgrounds of patients/families
  • Knowledge of applicable State/Federal laws governing hospice licensure
  • Ability to travel to multi-work locations during workday
  • Must have reliable vehicle, valid Texas Driver’s License and Liability Insurance
  • PC experience
  • Current CPR certification

Nice To Haves

  • Bachelor’s degree in nursing preferred
  • Five years minimum management experience, hospice/palliative experience preferred.

Responsibilities

  • Facilitate timely and complete admission, and continuing evaluation of patients for eligibility; assign staff to new admissions.
  • Assure the Plan of Care is updated, executed and effective with appropriate visit frequency.
  • Develop and promote positive relationships, acting as liaison between team members, patients, families, agency staff, and facilities to meet patient/family needs.
  • Perform ongoing review/assessment of team documentation for comprehensiveness, appropriateness, consistency, regulatory compliance and outcomes. Review findings with team and execute plan of correction.
  • Prepare for and facilitate scheduled IDT meeting.
  • Provide consultation and education for staff, health providers and community.
  • Maintain appropriate staffing pattern meeting productivity standards within established budgetary guidelines.
  • Assist with orientation of new staff/students; oversee staff, providing communication, training as needed.
  • Maintain required CEU credits according to established standards.
  • Understands and adheres to organizational compliance and privacy programs.
  • Assists in providing service that fosters the organization’s mission, values, vision and goals.
  • Performs other duties as required and/or requested by management.
