We are adding a Full Time Hospice Coordinator to the Springfield Phoenix family! Do you have a passion for making a difference in someone’s life? Requirements

  • Computer proficiency
  • Ability to work in a professional manner
  • Must work well both independently and in a group setting
  • Prior office experience
  • Professional appearance
  • Proficiency with Excel and Word
  • Problem solving skills and ability to multi-task
  • Excellent Organizational Skills
  • Efficient data entry skills

Responsibilities

  • Answer phones and coordinate details with pharmacy and DME partners.
  • Assist in management of IDT preparation.
  • Conduct Admission and Discharge audits.
  • Communicate with clinics, facilities, hospitals & other healthcare partners via phone.
  • Committed to providing excellent customer service.
  • Ability to work in a fast paced environment & have the ability to be flexible.
  • Assist Hospice Team members as needed with administrative tasks.
