About The Position

The Hospice Coordinator On-Call Registered Nurse serves as a member of the interdisciplinary team (IDT). Provides after-hour patient care that is consistent with the Plan of Care (POC) and is responsible for the provision of services that support the mission, vision, and shared values of Atlantic Health System/Atlantic Visiting Nurse (AVN) while meeting quality outcomes and regulatory standards. Principal Accountabilities: Provides skilled nursing care focusing on pain control and symptom management. Responds to medical or emotional crisis and/or death as requested by designated triage nurse. Conducts hospice admissions as needed. Consults appropriately with attending physician and medical directors to coordinate patient care and receive orders. Assists RNCMs with their patient assignments by conducting scheduled after-hours visits as necessary to meet patient care needs. Educates patients/caregivers on hospice philosophy, disease progression, physical components of care, stages of dying, and other aspects of the POC as necessary.

Requirements

  • Associated Degree in Nursing
  • BLS
  • Valid and unrestricted driver’s license with a clean driving record

Nice To Haves

  • Bachelor's degree in Hospice, Palliative Care, or related field.
