About The Position
The Hospice Community Specialist is responsible for developing and executing territory and account plans to maintain and grow hospice service admissions from referral partners within the designated service area. This role involves building relationships with community-based referral partners, including physician offices and senior living facilities, and assisting patients and families with hospice-related needs.
