About The Position

The Hospice Community Liaison at New Age Hospice is responsible for facilitating communication between caregivers, patients, and doctors, ensuring a positive relationship with referral sources through exceptional customer service. This role involves establishing relationships with healthcare professionals, managing patient referrals, and promoting the agency's services within the community.

Requirements

  • High School Diploma or Equivalent.
  • Minimum of one year prior experience as a Community Coordinator or in a comparable position.
  • Home nursing care experience preferred.
  • Ability to speak, read, and write in English.
  • Effective communication skills for face-to-face interactions and phone conversations.
  • Ability to interact and coordinate service delivery with staff and the community, including physicians, patients, and family members.
  • Current valid Texas Driver's license and proof of auto liability insurance.

Responsibilities

  • Facilitate communication between caregivers, patients, and doctors.
  • Provide exceptional customer service to maintain positive relationships with referral sources.
  • Demonstrate knowledge of state and federal regulations and guidelines.
  • Establish relationships with physicians to encourage patient referrals.
  • Obtain referral information on hospitalized patients and establish initial contact with patients and families.
  • Follow active patients during hospital stays or in long-term facilities.
  • Log therapy visits and submit referrals as received.
  • Handle patient-related phone calls appropriately.
  • Assist in obtaining physician orders as needed.
  • Serve as a liaison between staff and referral sources.
  • Travel to referral sources to perform job tasks.
  • Coordinate agency involvement in local health fairs and community programs.
  • Maintain compliance with company policies and regulations.
  • Prepare and proofread correspondence accurately.
  • Process mail and other correspondence daily.
  • Perform additional duties as requested by the supervisor.
