About The Position
The Hospice Community Liaison position at Full Hearts Hospice LLC in Houston, TX, is a full-time role focused on expanding the company's reach within the community and enhancing patient referrals. The primary responsibility of the liaison is to analyze the potential of the service area to identify target markets effectively. This involves visiting various healthcare facilities, including doctor's offices, hospitals, assisted living facilities, and skilled nursing facilities, to present the agency's credentials and secure patient referrals. The liaison will also conduct a thorough analysis of the company's organization to identify its strengths and weaknesses, as well as review past and current marketing data to inform future strategies. In addition to market analysis, the liaison will be responsible for developing sales and marketing objectives, creating a comprehensive marketing plan, and setting reasonable timetables for implementation. The role requires ongoing evaluation of the marketing plan and its effectiveness, with adjustments made as necessary based on established review periods. The liaison will also participate in interdisciplinary team (IDT) meetings as appropriate and assist office staff with administrative tasks, such as obtaining signatures from physicians and delivering updates. Community outreach and education are also key components of this position, ensuring that the agency's services are well-known and accessible to potential patients and their families.
